Moorland Community Care Group

 

Required as soon as possible: 

  • A Lead /Wellbeing coordinator approx. 18* hours/week @ £19150 per annum (pro-rata) for a fixed-term contract; duties apportioned as follows :-

Lead Wellbeing coordinator: Ashburton/Buckfastleigh (approx. 5 hours)

Wellbeing coordinator: Bovey Tracey (approx. 13 hours)

(* an additional 5 hours/week may be available until March 2017 to coordinate an existing Home from Hospital discharge project)

 The charity works with lonely isolated and vulnerable adults providing social care and is expanding its role within the NHS to provide guided help so that they can be proactive in managing their own wellbeing; reduce their need for urgent and emergency services by  improving their experiences within the community and being helped to focus on the things that matter to them and access community resources.

The post holder will be expected to work independently and initially will be directly responsible to the Board of Trustees.

The ideal candidate will have experience of working within the Health services or voluntary sector and be computer literate.  He or she will be sensitive to the needs and aspirations of clients and work closely with them to promote their own decision making.  An ability to work closely with multi-disciplinary teams and others in the group is essential.
